About the Role
As an RN Case Manager with us, you will collaborate closely with clinical teams to provide comprehensive case management for a specific patient population. Your role will involve managing complex medical conditions, socio-economic factors, and mental health co-morbidities. You will play a crucial role in optimizing patient health outcomes through assessment, planning, implementation, and ongoing evaluation of care options and services.
This is a fully remote position with occasional travel (10%-15%) required. Candidates must reside within 90 minutes of Houston.
Key Responsibilities
Conduct comprehensive assessments of healthcare and psychosocial needs for patients.
Develop, implement, and maintain individualized care plans aligned with internal protocols and accreditation standards.
Provide evidence-based clinical interventions based on care plan goals.
Collaborate with patient navigators to manage non-clinical aspects of care plans, such as scheduling appointments and arranging transportation or other patient needs.
Continuously monitor patient data and adjust care plans accordingly, including developing specific escalation plans for acute exacerbations.
Educate patients on their conditions and treatment options.
Serve as a patient advocate and facilitate access to necessary services and resources.
Maintain accurate documentation in electronic medical health records as per protocols.
Utilize evidence-based clinical protocols and guidelines to achieve defined quality metrics.
Requirements
Licensed Registered Nurse in good standing.
3+ years of RN Case Management experience in an outpatient setting.
CCM certification preferred, with an expectation to obtain within one year of employment.
Supervisory experience preferred.
Strong communication skills to interact empathetically with patients, families, and the interdisciplinary team.
Proficiency in computer skills, including multiple EMR and Case Management documentation systems.
Ability to meet accreditation and quality standards (e.g., NCQA, HEDIS).
Preferred Qualifications
Bilingual in English and Spanish is advantageous.

Compensation
The salary range for this role is $80k to $85k per year, paid bi-weekly. Final compensation decisions consider various factors such as skills, experience, education, and location.
